The PDF files below have been provided for clients and guests of clients. 

To view a map of a specific class location please go to the registration page and click on the current program of your choice and a map of the applicable location will be displayed.

If you are a current participant, and would like to bring a friend to participate for one class, please click on the "Bring a Friend" link below to download, print and fill in the required forms to hand in to your fitness instructor prior to participation. 

A completed "ParQ and Waiver" must be handed in the first day of class. All questions on the ParQ must be ansered as "NO" in order to participate. These forms are good for one year, after which, another form must be filled out if you continue to partcipate in our programs on an ongoing basis. If you have answered "YES" to any one or more of the ParQ questions, we require that you have your physician complete a ParMedX before you participate in any of our fitness programs.
